If you are a member of our team and plan to travel with us, each trip must have these forms filled out:

4H Field Trip Permission
Everyone on our team, adults, mentors, parents, students, that travel with us to a regional or state championship event sanctioned by FIRST, must fill out and turn in a media release consent form:
FIRST Media Release Consent Form = DUE in before you arrive to Boilermaker Regional, turn into Team President.
