If you are a youth in the 9th through 12th grade (6-8th ok too) looking to join our team, you’ll need to fill out and submit the following forms to join our team:

If your an adult, parent or engineer looking to help us mentor the youth in Huntington County, you’ll need to fill out and submit the following forms:

If you are a member of our team and plan to travel with us, each trip must have these forms filled out:

4-H Field Trip Permission
Media Consent is required for FIRST. Everyone on our team, adults, mentors, parents, students, that travel with us to a regional or state championship event sanctioned by FIRST, must fill out and turn in a media release consent forms. There are TWO ways to get this done.
